Clinical RN Case Manager for Schools

Join our team as a Registered Nurse Case Manager who empowers school staff with clinical care and training. This role focuses on supporting medically complex students in an educational environment.

As a dedicated RN Case Manager, you will work collaboratively with families, school personnel, and healthcare providers to deliver high-quality care. Your expertise will be essential in developing individualized care plans, monitoring student health, and providing education to staff on clinical practices. You will ensure that students receive optimal support and inclusive care.

Key Responsibilities:

• Conduct comprehensive nursing assessments and create individualized care plans

• Coordinate care with families, schools, and community providers

• Monitor and evaluate student health and care plans

• Lead educational training for school staff on clinical needs

• Ensure accurate documentation and promote quality improvement

Requirements:

• Graduate from an accredited nursing program

• Active/eligible CRNA registration

• Current BCLS-HCP certification

• 1–2 years as a school nurse or relevant experience

• Proficiency in electronic documentation systems
