As an RN Patient Care Manager, your role focuses on assessing and developing personalized care plans that meet client needs. Collaborating with clients, their families, and caregivers, you will manage client intake and service delivery while providing excellent customer service. This full-time position also includes periodic home visits and on-call responsibilities.

Key Responsibilities:

• Oversee client care and staff performance

• Manage client assessments and develop care plans

• Supervise caregiver matching and training

• Ensure accurate documentation and reporting

• Facilitate community networking for referrals

Requirements:

• Active CRNA registration as a Registered Nurse

• Minimum two years of clinical experience in home health

• Two years of direct supervisory experience

• Exceptional client assessment and communication skills

• Proficiency in Microsoft Office and software systems
