As an essential member of our interdisciplinary team, the Licensed Practical Nurse (LPN) reports to the Director of Care and delivers goal-directed nursing care.
Your role will involve collaborating with care teams, conducting resident assessments, implementing care plans, and providing hands-on nursing care to ensure residents' dignity and well-being.
Key Responsibilities:
• Collaborate with care teams using person-centered approaches
• Conduct assessments and maintain care plan documentation
• Implement and evaluate care plans as needed
• Assist with resident transitions and scheduling
• Respond to changes in residents’ conditions and manage emergencies
Requirements:
• Current registration as a Licensed Practical Nurse
• Experience in senior or long-term care preferred
• Strong clinical knowledge and communication skills
• Current CPR and First Aid certification
• Willingness to undergo Vulnerable Sector Check
