In this important role, Infinity Health seeks an experienced Registered Nurse to implement wellness assessments and mental health screenings for seniors in their homes. You will be responsible for conducting comprehensive evaluations, care planning, and ensuring safety in patients’ living environments. The position allows for independent practice while collaborating within a network for continuity of care.
Key Responsibilities:
• Travel to seniors' homes within Calgary for assessments
• Conduct holistic screenings for mental and physical health
• Evaluate living environments for safety risks and vulnerabilities
• Develop care plans and recommend resources for seniors
• Maintain collaborative communication with the practice network
Requirements:
• Active RN registration with CRNA
• Background in Mental Health or Geriatric care
• Experience with formal clinical assessments
• Strong clinical judgment and critical thinking skills
• Valid driver’s license and reliable vehicle for travel
