In this essential role, the Coordinator will educate patients, families, and health care providers about MAiD services. You'll collaborate with a multidimensional team including nurses and physicians while leveraging your planning and organizational skills. This position requires a compassionate self-starter capable of working under pressure and meeting tight deadlines in a complex landscape of end-of-life care.
Key Responsibilities:
• Provide information about MAiD services to various stakeholders
• Collaborate with healthcare teams to facilitate MAiD
• Organize and coordinate patient navigation and care delivery
• Conduct assessments and establish rapport with patients
• Demonstrate effective communication and problem-solving skills
Requirements:
• Bachelor’s degree in Nursing
• Registered or eligible with CRNA
• Minimum 3 years in community palliative care
• Experience with case management
• Knowledge of MAiD legislation
