Permanent Full-Time (1.0 FTE) Primary Care Nurse Practitioner
Peaks to Prairies Primary Care Network (PCN) is a physician-led non-profit corporation that works collaboratively with Alberta Health Services and our community partners to support primary care in Olds, Sundre and surrounding area. Our mission is to be a health home where we provide patient-focused care, support, and resources to enable our patients to achieve their best health.
Are you a Nurse Practitioner who meets the competency requirements and standards for working at full scope of practice? If so, we invite you to apply for this opportunity to join our PCN! The position will be a Permanent 1.0 FTE and will be located at the Moose & Squirrel Medical Clinic in Sundre.
As a member of an interprofessional team in the patient medical home, the Nurse Practitioner will work collaboratively with physicians and team members to provide high quality, evidence-based primary health care to an assigned patient population of all ages. There will be a primary focus on patients with complex chronic health concerns.
Our ideal candidate has:
- Demonstrated experience in successfully working as part of an interdisciplinary team to provide patient and family-centered care in the patient medical home.
- Previous Nurse Practitioner experience in primary health care setting.
- Advanced organizational, prioritization and panel management skills.
- The ability to take initiative and be a self-starter and self-learner.
- Decision making, strategic and critical thinking skills.
- Strong relationship building skills with the team, patients, and their families.
- Previous experience in using electronic medical records.
- A Master of Nursing with a Family Nurse Practitioner focus.
- Current registration with CARNA.
Salary:
