As part of the Adults Team in Integrated Home Care, you'll serve clients and families by assessing needs, coordinating services, and promoting continuity of care. This position requires you to liaise with various healthcare facilities, providing nursing interventions tailored to a select group of clients. Your leadership will support a holistic approach to patient care, health promotion, and overall well-being.
Key Responsibilities:
• Assess needs of clients and their caregivers
• Plan and implement nursing interventions
• Collaborate with healthcare providers and organizations
• Review and evaluate client progress and care plans
• Deliver health education and advocacy for families
Requirements:
• Accredited nursing education and active CRNA registration
• 1-2 years of Home Care experience preferred
• Current Basic Cardiac Life Support certification
• Strong critical thinking and time management skills
• Valid driver's license and vehicle for travel
