As a Licensed Practical Nurse (LPN) at Age Care, you’ll report to the Director of Care and provide goal-directed, patient-focused nursing within a collaborative, interdisciplinary team.
Your role involves conducting resident assessments, implementing care plans, and educating residents and families about health conditions. You'll actively participate in care conferences and assist with resident transitions, ensuring high-quality care delivery.
Key Responsibilities:
• Conduct accurate resident assessments and maintain documentation
• Implement and evaluate personalized care plans
• Educate residents and families on health management
• Administer medications and treatments per regulations
• Respond to changes in residents’ health conditions
Requirements:
• Current LPN registration in British Columbia
• Experience in senior or long-term care is preferred
• Strong clinical and communication skills
• Current CPR and First Aid certification
• Willingness to complete a Vulnerable Sector Check
