• Collaborate with the practice network for care continuity

Requirements:

• Registered Nurse status with CRNA

• Experience in Mental Health or Geriatric care

• Ability to perform formal clinical assessments

• Strong independent judgment and critical thinking

• Valid driver’s license and reliable vehicle for outreach
