Age Care is seeking a skilled Licensed Practical Nurse (LPN) to enhance our team. Key Responsibilities:

• Conduct resident clinical assessments and document care plans

• Implement and evaluate care/service plans effectively

• Assist in scheduling and participating in care conferences

• Provide direct resident care and administer medications

• Communicate closely with the Director of Care for transitions

Requirements:

• Diploma in Practical Nursing

• Active registration in good standing in the province

• Over 3 years' experience in seniors' health

• Current CPR certification

• Proficiency in Microsoft Office applications
