Reporting to the Supervisor of Patient Access, the Primary Care Nurse will provide comprehensive nursing services at our clinics. You will evaluate patients' primary and complex health needs, implement care plans, and engage in health promotion activities. This is an opportunity to work with interdisciplinary teams to improve patient outcomes and foster wellness in the Calgary communities.
Key Responsibilities:
• Assess patients’ health and well-being at multiple levels
• Coordinate management for chronic disease patients
• Utilize motivational interviewing techniques for support
• Document patient care in Electronic Medical Records
• Engage in community outreach activities for health promotion
Requirements:
• Bachelor’s degree in Nursing required
• Licensed with College of Registered Nurses of Alberta
• Current CPR certification
• Experience with diverse, high-risk populations
• Minimum of 2 years relevant nursing experience preferred
