As a Licensed Practical Nurse in Home Care, you'll be part of an inter-professional healthcare team, assessing and providing direct nursing care across diverse settings. With a focus on client strengths and self-care goals, your contributions help shape personalized care plans that evolve based on client responses. Expect a mobile role, delivering care in client's homes and the community.
Key Responsibilities:
• Assess and evaluate client needs collaboratively with RN Manager
• Provide person-centered direct nursing care across all ages
• Develop and adjust care plans based on ongoing assessments
• Work flexibly as a mobile caregiver in various locations
• Facilitate communication among clients, families, and healthcare team
Requirements:
• Accredited practical nurse education program completion
• Current CLPNA registration or eligibility
• Basic Cardiac Life Support certification
• Post-basic Medication Administration Course completion
• Valid driver's license and reliable vehicle access
