If you enjoy a role in providing services to clients in community-based settings in rural communities, then this position will be of interest to you. The Licensed Practical Nurse (LPN) in Home Care works as a member of an inter‑professional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span.

The LPN is responsible for her/his nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). In collaboration with the Registered Nurse Case Home Care Manager, the LPN assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families.

Using a person‑centered approach, you develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self‑care goals, and provide direct nursing care. In collaboration with the client/family, you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization.

There is also an expectation to be a mobile worker, including caring for clients in their homes or in the community.

The position is approximately 64 km South‑West outside of Calgary City Limits. The Rural South team is comprised of Diamond Valley, Okotoks, High River, Nanton, Claresholm and Vulcan. The farthest site is 160 km south of the city.

Responsibilities

Assess, plan, implement and evaluate home care client needs in collaboration with the RN Case Home Care Manager and the client/family.

Provide direct nursing care to individuals across the life span using a person‑centered approach.

Develop and update care plans based on client response to interventions and stewardship principles.

Serve as a mobile worker, delivering care in client homes and community settings.

Facilitate communication and build relationships with clients, families, and the inter‑professional health care team.

Maintain nursing practice in alignment with the College of Licensed Practical Nurses of Alberta (CLPNA) standards and the Continuing Care Case Management Competency Framework.

Required Qualifications

Completion of an accredited practical nurse education program.

Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA).

Current Basic Cardiac Life Support – Health Care Provider (BCLS‑HCP).

Additional

Required Qualifications

Completion of a post‑basic Medication Administration Course for LPNs.

Ability to carry and move equipment, use a laptop computer and mobile phone.

Manual handling capability including lifting, lowering, carrying, pushing and pulling up to 50 lbs.

Possession of a valid driver’s license and access to a motor vehicle in good repair for work‑related purposes.

Preferred Qualifications

Minimum of two (2) years of current nursing experience.

Recent work experience in a community‑based setting.

Home Care experience preferred.
