Registered Nurse and Case Manager

Provide compassionate nursing care as a Registered Nurse and Case Manager with AHS in Calgary. Focus on holistic patient support and integrated care in client homes and communities.

AHS is seeking a Registered Nurse who will lead in patient safety and well-being.

Your role as a Case Manager will involve developing person-centered care plans while ensuring quality nursing assessments. You will integrate care with clients and their families, adapting services to meet evolving health needs.

Key Responsibilities:

• Deliver patient-centered nursing care in community settings

• Develop individualized care plans focusing on client strengths

• Provide case coordination and ongoing assessments

• Collaborate with interdisciplinary teams for holistic care

• Maintain accurate documentation and reporting of services

Requirements:

• Accredited nursing program completion with 2 years experience

• Current registration with CRNA or eligible for practice permit

• Valid driver’s license and vehicle access for travel

• Current BCLS-HCP certification within the last year

• Experience in Home Care and case management preferred
