Active participation in coordinating home health services is crucial to ensure patient needs are met.

Key Responsibilities:

• Provide direct patient care within the Nurse Practice Act

• Implement registered nurse's care plans effectively

• Document care activities accurately and timely

• Educate patients and families on diagnosis and care

• Coordinate home health services and report needs

Requirements:

• Graduate from an accredited practical or vocational nursing program

• Three years nursing experience preferred

• Current LPN/LVN licensure in the state

• Strong communication and organizational skills

• Basic computer proficiency
