Job Summary:

The Nurse Practitioner is a leadership role within a multidisciplinary and inter-professional team. This Nurse Practitioner role in NLOT team is primarily responsible for providing clinical care to support Long Term Care (LTC) homes. or congregate settings. The NP works in collaboration and consultation with a physician, the LTC or congregate setting coordinators, and members of the inter-professional health care team to respond to facilitate hospital discharge, and prevent ED visits.

The NP will act as a link between hospital and the LTC homes to ensure the most appropriate and efficient discharge.

Responsibilities:

  • Provision of Nurse Practitioner scope of clinical care for LTC or congregate setting residents in accordance with the College of Nurses (CNO) Practice Standard for Nurse Practitioners and all other applicable legislation, including assessment, ordering and interpreting diagnostics, prescribing pharmacological agents and therapeutic measures, and performing controlled acts
  • Collaboration with hospital and LTC teams to assist in identifying potential barriers and supporting with discharge strategies to reduce avoidable ED visits, decrease LOS, and minimize readmissions
  • Capacity building for LTC staff to develop and enhance competency for acute treatment protocols
  • Integration of best practice guidelines and current evidence-based research into clinical practice for a select patient population
  • Evaluation of outcomes for the programs and make recommendations for change in consultation with Program Director and Manager
  • Development of linkages with external partners as appropriate to ensure continuity of care
  • Enhance communication between hospital and LTC to support the plan of care transitions in care facilities
  • Addressing medication reconciliation issues upon discharge from hospital
  • In keeping with professional standards of practice, obtains and/or disseminates only that information which is relevant to meeting the client’s identified needs and with appropriate legal authorization, either in the form of signed consents or direct authorization from the client or designated substitute decision-maker.
  • Utilization of expert communication skills by being a link between the client, consultants, resources and the referral source which may be required to coordinate the provision of service to the client.
