This position, reporting to the Director of Care, emphasizes a harm reduction approach while providing comprehensive support to clients and families. You will lead nursing processes in accordance with TPFA and CLPNA guidelines, provide direction to Health Care Aides, and respond effectively to client needs and concerns.
Your role includes documentation of nursing procedures and assessments as well as collaboration with interdisciplinary teams.
Key Responsibilities:
• Lead nursing processes per established policies
• Direct Health Care Aide assignments for efficiency
• Conduct detailed nursing assessments of clients
• Document care interventions and outcomes accurately
• Coordinate care conferences involving families and clients
Requirements:
• Graduated from an LPN program, eligible for CLPNA
• Basic Cardiac Life Support certification necessary
• Experience in supportive living is ideal
• Knowledge of RAI MDS and Point Click Care preferred
• Excellent communication and organization skills
