In this vital role, the Home Health RN will provide skilled care directly in participants' homes, empowering them to maintain their independence. Key Responsibilities:
• Assess nursing equipment needs during home visits
• Participate actively in interdisciplinary team discussions
• Execute and document nursing interventions as per care plans
• Educate families on disease processes and self-care
• Coordinate scheduling with the Home Health Manager
Requirements:
• Graduate from an accredited nursing school
• Valid, unencumbered RN License required
• One year of clinical experience with elderly populations
• Reliable transportation available for home care visits
• Obtain CPR and First Aid certification within 180 days
