As a Licensed Practical Nurse, you will play a key role in the Consult and Treat Team, responsible for case management functions and direct patient care. With your experience and advanced judgment, you'll coordinate care planning and deliver high-quality services to both short-term and long-term clients. Your impact is crucial in promoting patient family-centered care within a collaborative multi-disciplinary environment.
Key Responsibilities:
• Conduct nursing assessments and guide care planning
• Provide direct clinical care in community settings
• Monitor service delivery and client outcomes
• Collaborate with multidisciplinary teams for patient care
• Ensure adherence to policies and procedures
Requirements:
• Completion of accredited practical nurse education program
• Active registration with College of LPNs of Alberta
• Minimum 2 years current LPN experience
• Basic Cardiac Life Support certification
• Extensive knowledge of ethical principles in home care
Support client health through effective case management and clinical excellence as part of Assisted Living Alberta.
