Mosaic Primary Care Network seeks a Temporary Full-Time Primary Care Nurse to provide vital healthcare services in Calgary. Focus on patient-centered care, chronic disease management, and health promotion within a supportive team.

As a Primary Care Nurse (RN) at Mosaic PCN, you will play an essential role in assessing and managing patient care for various health conditions. This position requires collaboration with physicians and community members to develop personalized care plans. You will also engage in health education and community outreach efforts.

Key Responsibilities:

• Assess patients' primary health care and chronic needs

• Facilitate care coordination and case management

• Teach and advocate for disease prevention strategies

• Monitor patient health status and adjust plans as needed

• Document patient interactions in electronic records

Requirements:

• RN with Bachelor’s degree and CRNA license

• Two years of nursing experience, preferably in primary care

• Demonstrated clinical skills and motivational interviewing

• Strong interpersonal and collaborative skills

• Flexibility in working hours required
