In this role, you will be responsible for filling in during staff absences while providing quality nursing care aligned with the needs of clients across multiple locations. You’ll utilize your critical thinking and organizational skills to create effective care strategies while ensuring safety and patient advocacy. Your commitment to holistic care will be essential in supporting vulnerable populations.
Key Responsibilities:
• Perform nursing assessments and care coordination
• Deliver direct nursing services in clients’ homes
• Interact with families to support ongoing care
• Work with multidisciplinary teams for effective service
• Monitor client responses and adjust care plans accordingly
Requirements:
• Accreditation from an approved nursing program
• At least 5 years of nursing experience, including community care
• Proficient in computer use and communication
• BCLS certification from the Heart and Stroke Foundation
• Ability to lift at least 30 lbs
