In this pivotal role, you will report directly to the Director of Care, working collaboratively with an interdisciplinary team to ensure that each resident receives high-quality and individualized care. Your efforts will help maintain a safe and dignifying atmosphere for residents and their families. Responsibilities include creating service plans, administering care, and assisting through palliative stages.

Key Responsibilities:

• Implement and modify personalized care plans

• Maintain comprehensive health records for residents

• Assist with palliative care and ensure dignity in end-of-life transitions

• Provide direct nursing care, including medication distribution

• Guide Health Care Aides in best care practices

Requirements:

• Graduate from an accredited nursing program

• Valid nursing license and CPR certification

• Experience with geriatric or mental health populations

• Strong teamwork and mentorship skills

• Ability to handle emergency situations
