This position entails direct reporting to the Director of Care and playing an essential role in an interdisciplinary healthcare team. As a Licensed Practical Nurse, you will carry out detailed client assessments and create tailored care plans that address varied needs. Additionally, you'll oversee medication administration and ensure comprehensive monitoring of client conditions to achieve optimal health outcomes.
Key Responsibilities:
• Conduct thorough client assessments for tailored care
• Develop care plans supporting all client needs
• Administer and supervise medication delivery
• Monitor client health and report status changes
• Collaborate with health care professionals in teams
Requirements:
• Completion of a recognized LPN program
• Active registration with CLHA required
• Basic Life Support CPR certification necessary
• Preference for candidates with nursing experience
• Leadership skills must be demonstrated
