Reporting to the Supervisor, Urban Response, the Licensed Practical Nurse (LPN) works in Urban Response programs, which include the Episodic Medical Care Clinic and Rapid Response programs. These programs support clients utilizing Emergency Shelter services that are in various stages of their housing and recovery journeys. The Episodic Medical Care Clinic program provides episodic medical and social support(s) to clients experiencing homelessness and acute medical concerns while the Rapid Response Team provides emergency response and short-term medical observation to individuals who have recently experienced a medical emergency and require additional supports or intervention.

The Licensed Practical Nurse (LPN) utilizes skills and knowledge to provide care for our clients in multiple service areas within the Urban Response programs. This role is a part of the integrated Health & Recovery Services team and is expected to work in both our Episodic Medical Care Clinic, which may include the Medication Assistance program and Nurse Led Walk-In Clinic at times, and the Rapid Response Team.

The Licensed Practical Nurse may perform tasks related to emergency response, applying various standardized assessment tools, medication assistance, symptom management, wound care, documentation, health promotion, referrals to internal and external resources, mental health and crisis intervention, and other tasks, as assigned by Health & Recovery Services Leadership. In some extreme cases, the Urban Response LPN may be called upon to provide nursing care to clients within the Recovery Oriented System of Care programs from time to time.

48 hours per rotation with 4 days on and 4 days off. Some unusual hours may be required from time to time as the need arise. Schedule may be altered at the discretion of management.

• Perform delegated independent nursing functions within the scope of practice as determined by CLHA and the DI within the individual LPN competence level, as observed by the Health & Recovery Services Leadership team.
• Assist with other frontline staff duties when time allows.
• Consistently demonstrate the ability to take the initiative to make decisions/choices without direct supervision.
• Be informed of APL routines and maintains standards and knowledge around phlebotomy and lab services.
• Participates in formal and informal health teaching including preventative health concepts concerning Hepatitis A, B, C, HIV, condom usage, nutrition, STBBI’s, basic…
