Date Available: Negotiable

Hours of Work:7.75 hours per day, 38.75 hours per week. Availability to work evenings and weekends is required.

Position Summary

The Foot Care Nurse (LPN) provides preventative foot care and patient education, supporting patients developing self-management skills and preventing foot-related complications. This team-based, short-term program is designed for low-to moderate-risk patients and focuses on preventative foot care, education, and self-management support.

The Foot Care Nurse provides foot care and non-complex lower leg assessments; non-invasive interventions, nail trimming/filing, corn/callous treatment; self-management teaching. The Foot Care Nurse provides a high quality of evidence-based health care to the practice population and in accordance with the standards defined by the College of LPNS’ and HCAs of Alberta (CLHA).

Key Responsibilities Assessment
  • Assesses and identifies the real and potential risks in order to optimize foot care health and well-being.
  • Assesses footwear and gait analysis.
Intervention
  • Utilizes health coaching/case management approaches when assessing, planning, implementing, and evaluating foot care needs of the patient.
  • Utilizes motivational interviewing techniques and behavioral change management to provide support, advice and additional resources to empower patients to achieve goals in health behaviour changes, and ultimately self-management of foot care.
  • Provides comprehensive assessments and health screening.
  • Teaches, councils, and advocates to enhance health and well-being with an emphasis on prevention of foot care problems and/or diabetes disease progression.
  • Participates in quality improvement development at the clinic level.
  • Monitors the assessment data, the ongoing health status of the individual/family, recognizes changes in health care needs, and adjusts the care plan accordingly.
  • Completes a full lower leg assessment (skin and nails or feet, bony prominences, sensation testing using a 10-g monofilament, checking blood flow and pedal pulses, wound and/or infection assessment).
  • Treats low to moderate risk foot issues with non-invasive procedures: ingrown toenails, callouses, corns, and other foot complications related to diabetes.
  • Creates individualized treatment plans focusing on patient goals and health education
  • Empowers patients to self-manage their foot care.
Multidisciplinary Collaboration
  • Mentors’ colleagues and nursing students in areas of expertise and seeks mentorship to achieve full potential in professional development.
Documentation
  • Documents all phases of patient care in the appropriate Electronic Medical Record (EMR) to maintain accurate and timely records, as well as recording the details of clinical interactions in MPCN evaluation systems.
