Provide autonomous geriatric care at South Health Campus using Comprehensive Geriatric Assessments and advanced clinical reasoning. Work primarily on day shifts.
As a Geriatric Nurse Practitioner with Acute Geriatrics, you will assess older adults with frailty, complex comorbidities, and geriatric syndromes. You will integrate nursing and medical knowledge to diagnose, order and interpret tests, prescribe treatment, and perform procedures. Your practice supports patients, families, and healthcare teams across acute care.
Key Responsibilities:- Conduct Comprehensive Geriatric Assessments for referred acute care patients
- Develop, implement, and evaluate individualized plans of care
- Prescribe therapies and coordinate care across acute settings
- Provide consultation and follow-up within inpatient units and emergency departments
- Support education, leadership, research, and quality improvement initiatives
- Approved Nurse Practitioner program and minimum Master's degree
- Active CRNA Nurse Practitioner Registrar registration and provisional licensure
- Current Basic Cardiac Life Support Health Care Provider certification
- Two years of geriatric or relevant acute care experience
- Two years of Nurse Practitioner experience, or recent qualifying practicum
