In collaboration with the Registered Nurse Case Manager, the LPN is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families. Required Qualifications:
Completion of an accredited practical nurse education program.
Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA).Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional
Required Qualifications:
3 to 5 years of Nursing experience, including 1 to 2 years community experience. Demonstrated clinical assessment skills, ability to provide direct nursing interventions including HPTP, wound care, management of tubes lines, drains, medication management, mobility and transfers, interdisciplinary coordination. Demonstrated ability to operate within an electronic health record.
Proven ability to work in an interdisciplinary team.
Good negotiation and conflict resolution skills.
Knowledge of ethical principles and their application in the clinical/home setting.
Physically able to perform all assigned tasks.
This role requires travel; possession of a valid drivers' license and access to a motor vehicle in good repair for work related purposes as a requirement of this position.
You will be required to operate a laptop computer and mobile phone, and for handling (lifting, lowering, carrying, pushing, and pulling) as a regular part of the job. For example: lifting approximately 30 pounds in an out of a vehicle on a regular basis and carrying this weight up and downstairs throughout the day is common within this job.
Intermediate level of computer skills.
Preferred Qualifications:
Experience with mental health and/or addictions/harm reduction, palliative care, chronic disease management.
