Senior Care Licensed Practical Nurse

Support seniors’ health at Age Care Dawson through goal-directed nursing care, care planning, and treatment administration. Work in Chestermere on a regular part-time basis.
This experienced Licensed Practical Nurse works with an inter-professional team under the Director of Care. You will apply Age Care’s person-centered Professional Practice Model. The role includes evaluating care plans, communicating changes, and coordinating resident conferences.

Key Responsibilities:

Apply the person-centered approach within inter-professional practice
Maintain current resident care/service plans and records
Communicate assessment and care changes as required
Assist with resident care conferences and participate where applicable
Document care according to regulatory standards and policies
Requirements:
Practical Nurse Diploma
Current registration in the applicable province
Membership in good standing with the applicable province
Three-plus years supporting seniors’ health
Current CPR training and computer proficiency
Use senior care experience, care planning, and clinical documentation skills to support Age Care residents and their families.
