In this role, you'll address patient needs by assessing, advising, and ensuring seamless referrals across Dialogue's integrated health services. Your expertise in clinical assessment and customer relations will drive positive health outcomes for diverse populations. We're looking for dedicated professionals who are adaptable and compassionate.
Key Responsibilities:
• Evaluate patients using video and phone consultations
• Direct patients to suitable health services based on assessments
• Advise on at-home symptom management strategies
• Prescribe medications in compliance with virtual practice standards
• Document all interactions and provide follow-up care
Requirements:
• Minimum 2 years’ nursing experience necessary
• Must hold a valid nursing license in Ontario or Quebec
• Knowledge of primary care practices is advantageous
• Experience in community or emergency care preferred
• Availability for evening and weekend shifts required
