This full-time role focuses on providing personalized case management for clients facing barriers to employment. The position requires the completion of client intake processes and the development of Individual Service Plans. You will actively liaise between clients, employers, and support networks to ensure successful outcomes.
Key Responsibilities:
• Complete client intake and ensure eligibility
• Conduct Person Centered Plan interviews with clients
• develop and monitor Individual Service Plans
• Collaborate with employers to set work goals
• Build relationships with community and business partners
Requirements:
• Minimum 2 years post-secondary education in related field
• 2 years experience working with individuals with disabilities
• Valid First Aid certificate
• Clean driving record and own vehicle
• Excellent oral and written communication skills
