Family Intensive Case Managers play an integral role in CUPS’ approach to care, guiding families with complex needs toward their goals. You will assist to create care plans tailored to the unique strengths and struggles of each family unit, utilizing resources and building community connections to promote overall well-being. Your efforts will help create a supportive environment that fosters growth and stability.
Key Responsibilities:
• Attend case conferences and home visits for comprehensive support
• Lead programs in Resource Room for skill-building
• Collaborate with internal and external agencies
• Assist families in creating and updating care plans
• Monitor and assess client progress regularly
Requirements:
• Diploma or Degree in social work or similar field
• Minimum 3-4 years of relevant experience
• First Aid and CPR certification required
• Strong grasp of Calgary's support services
• Excellent communication skills in English
