The CCMS Caseworker plays a vital role in helping individuals through the Community Case Management and Supervision program. This position requires engaging with clients, conducting file reviews, and creating comprehensive release plans. You'll provide necessary documentation and empathetic support while managing crisis situations, all to facilitate community reintegration.
Key Responsibilities:
• Engage effectively with ethno-cultural community members
• Conduct thorough file reviews to develop release plans
• Complete case documentation and funder reports
• Support clients facing trauma with guidance strategies
• Arrange activities to encourage community engagement
Requirements:
• Post-secondary education in human services or related field
• Minimum two years of Intensive Case Management experience
• Knowledge of corrections as an asset
• Brain Story Certification preferred
• Valid driver's license and personal vehicle required
