This permanent position focuses on empowering vulnerable individuals through support in housing stability, financial access, and skill development. Collaborating closely with Transition Specialists, you will be part of a team dedicated to facilitating successful community integration while enhancing residents’ overall well-being.
Key Responsibilities:
• Teach essential life skills to promote independence
• Model and support individuals in maintaining health
• Monitor and ensure compliance with safety standards
• Deliver comprehensive service from intake to transition
• Execute thorough assessments and care planning
Requirements:
• Degree or diploma in Human Services essential
• At least three years’ experience with at-risk populations
• Exceptional organizational and interpersonal skills
• Knowledge of the Homeless Serving System preferred
• Must hold valid Driver’s licence and ensure background checks
