Community Resource Coordinator at Easter Seals

Easter Seals Alberta is looking for a Community Resource Coordinator who will enhance lives by supporting individuals with disabilities. This full-time role focuses on resource management and family advocacy.

In this position, you will support individuals and their families in navigating community resources and services. Your responsibilities will include conducting needs assessments and assisting with applications related to income supports and adaptive equipment. You will foster a culture of inclusion and respect while ensuring accessibility to necessary supports.

Key Responsibilities:

• Provide direct service and case management for clients

• Facilitate needs-based assessments and service plans

• Refer clients to essential community resources

• Maintain meticulous client service documentation

• Offer support during crisis situations and referrals

Requirements:

• Relevant diploma/degree in Social Work desired

• Minimum 2 years in resource navigation and assessments

• Strong interpersonal and communication skills

• Experience with disabilities and chronic conditions

• Ability to relate to diverse community members
