This part-time position requires a professional with a Social Work degree and registration with the Alberta Association of Social Workers. You will leverage your expertise and two years of experience to assist clients and families in navigating financial and social issues. Collaboration with the Community Care Coordinator is vital to ensure effective discharge planning and community transition.
Key Responsibilities:
• Support clients dealing with social and financial issues
• Develop care discharge plans in collaboration with teams
• Provide updates and support to clients and families
• Conduct thorough assessments for care needs
• Facilitate community return processes for clients
Requirements:
• Degree in Social Work or an equivalent
• Registration with the Alberta Association of Social Workers
• Minimum 2 years in clinical and continuing care
• Experience in gerontology is advantageous
