Transition Specialist

Position: Full-Time Transition Specialist Role
Join McMan Calgary as a Transition Specialist and provide essential support to individuals leaving homelessness. This full-time role focuses on developing crucial stability and housing plans.

Your primary responsibility will be to serve tenants of the James House program, ensuring they have the necessary support to thrive independently. By utilizing trauma-informed and harm reduction strategies, you will aid clients in their move to stable living while maintaining a collaborative team approach. Your work will make a tangible impact on their journey to self-sufficiency.

Key Responsibilities:

• Provide client-focused, strengths-based assistance

• Establish trusting relationships with clients and networks

• Complete individualized service planning processes

• Facilitate crisis and safety plans with clients

• Participate in program quality assurance efforts

Requirements:

• Degree or diploma in Human Services

• At least three years’ experience in similar roles

• Proficiency in organizational and interpersonal skills

• Knowledge of community resources and services

• Valid driver’s license with insurance coverage
