Transition Specialist at McMan

Elevate lives as a Transition Specialist with McMan Calgary & Area. This full-time position emphasizes client-centered support for individuals exiting homelessness and establishing stable housing.

As a Transition Specialist, you will engage with tenants from the James House program, helping them develop service plans and natural support networks. Using strength-based practices, you’ll assist clients during their journey from homelessness to independent living, ensuring a smooth transition and the necessary support. Your work will redefine stability for those in need.

Key Responsibilities:

• Deliver client-centered support through various therapeutic approaches

• Build trusting relationships with clients and support networks

• Complete strengths and cultural discovery for individual service planning

• Facilitate collaborative planning for crises and safety

• Contribute to program evaluation and Wraparound standards

Requirements:

• Degree or diploma in Human Services

• Three years’ experience with vulnerable populations

• Strong organizational and interpersonal skills

• Knowledge of community resources and services

• Valid Driver’s license with insurance coverage
