The Case Manager works directly with young adults in a supportive housing setting, focusing on creating individualized support plans that cater to their specific needs. This role encompasses connecting clients with community resources, managing health and educational services, and advocating for their well-being and rights. You will also engage in their day-to-day lives, offering in-house support and guidance.
Key Responsibilities:
• Initiate connections to essential support services
• Assist tenants in navigating health and education options
• Conduct group workshops and support meetings
• Provide ongoing advocacy for tenant needs
• Directly support tenants in daily routines
Requirements:
• Degree or diploma in a relevant field
• 3-5 years related experience with youth at risk
• Current criminal record and intervention checks
• First Aid and CPR/AED certification necessary
• Class 4 or 5 Driver’s License mandatory
