As a Case Manager, you will play a vital role in coordinating care and facilitating access to resources for clients during their critical healing journey. Your responsibilities will include supporting clients through active listening and crisis intervention while maintaining a client-centered approach within a multidisciplinary setting. Expect to be dynamic and adaptable as you respond to diverse client needs.
Key Responsibilities:
• Engage clients in identifying and accessing necessary resources
• Facilitate both group and individual client programs
• Ensure integrity in client documentation and service delivery
• Work collaboratively with community partners and agencies
• Provide advocacy alongside clients in navigating support systems
Requirements:
• Bachelor’s degree in social work or a related field
• Minimum 2-3 years experience with vulnerable individuals
• 2+ years of direct case management
• Strong knowledge of available community resources
• Valid registration under the Health Professionals Act in Alberta
