Case Manager for Substance Use Services

Join Green Shield as an Assessment and Treatment Case Manager, enhancing the customer experience in substance use services. Drive impactful coordination of assessments and treatment while supporting client compliance and progress.

In this role, you will focus on improving assessments and post-treatment services for clients. As an Assessment and Treatment Case Manager, you will monitor client engagement, facilitate treatment referrals, and manage necessary documentation. Your work will directly contribute to a more holistic health approach that benefits all Canadians.

Key Responsibilities:

• Facilitate clinical service setup for assessments and treatments

• Monitor attendance and progress towards recommendations

• Prepare reports for Clinical Manager review

• Provide regular updates to customers on case status

• Collaborate with Clinical Managers for efficient case management

Requirements:

• Bachelor’s Degree in Social Work, Social Sciences, or Psychology

• 3-5 years of experience in case oversight

• Experience in substance use disorders is an asset

• Strong communication and organizational skills

• Proficiency in Google Suite and Microsoft Office
