Case Manager for Housing Supports

Join Inn from the Cold as a dedicated Case Manager, focusing on intensive case management for families. Support participants in securing housing and achieving sustainable independence within the community or at Neoma.

As a Case Manager within the Housing with Recovery Supports team, you'll manage a caseload of 15 families in need.

Your role emphasizes creating strength-based case plans while providing on-going advocacy and resource navigation. You will meet families in various settings to monitor their progress and refine goals to address challenges.

Key Responsibilities:

• Support families in sourcing housing options

• Develop participant-led case plans aligned with goals

• Meet regularly with families for progress support

• Provide advocacy for accessing community resources

• Maintain accurate, confidential case documentation

Requirements:

• Diploma or degree in social work or human services

• Minimum 2 years in family case management

• Experience with vulnerable populations

• Knowledge of trauma-informed care principles

• Valid driver’s license and reliable transportation
