This full-time role based in Calgary, Alberta, focuses on seniors transitioning from medical care. You will initiate client contact, provide hands-on support, and coordinate various community services. Your expertise in social work and understanding of senior-specific needs will be crucial in fostering connections and ensuring well-being.
Key Responsibilities:
• Conduct in-home client assessments and identify needs
• Develop short-term transition plans for seniors
• Provide transportation and grocery assistance post-discharge
• Advocate for client needs while maintaining independence
• Monitor progress and adjust plans as necessary
Requirements:
• Degree or diploma in Social Work or related field
• 3+ years in case management or community support
• Strong understanding of senior needs and transitions
• Excellent communication and planning skills
• Valid driver’s license and reliable vehicle
