Position: Case Manager - HRS )

Position Overview

We are seeking a passionate and dedicated Case Manager to join the Housing with Recovery Supports team at Inn from the Cold. The Case Manager, HRS will manage a caseload of 15 families housed either in the community or at Neoma; families typically remain in the program for up to two years. The role involves providing intensive case management supports, assisting families from sourcing housing through the development of case plans to achieving sustainable independence.

What

You’ll Do
  • Support participant families in sourcing housing – either in designated units at the Neoma building or in the community.
  • Assist participants in developing a strength‑based case and participant‑led case plan to address needs and attain goals over time.
  • Meet frequently with families (in‑home and in the community) to provide support and monitor progress.
  • Provide ongoing advocacy and resource navigation support to address barriers and assist families in accessing culturally relevant community resources, employment services, mental health support, health care, and financial assistance.
  • Review and refine goals with participants regularly, utilizing assessment tools (Outcome Star and Service Plan); adjust goals and actions as needed during crises or significant changes.
  • Maintain accurate, timely, and confidential documentation of family interactions, case notes, intake and assessment information, safety concerns, and records of support and service referrals using HOMES and HMIS databases, and the physical participant file.
  • Work both physically in the office and in the community; frequent travel within the city may be required.
Qualifications
  • Completion of a diploma or degree program related to social work, family case management, or human services is preferred but a combination of education, experience, and training will be considered.
  • At least 2 years of experience in family case management, family support, and/or crisis support.
  • Experience working with vulnerable populations experiencing homelessness.
  • Knowledge of trauma‑informed and participant‑centered care and conflict resolution skills.
  • Understanding of issues related to homelessness and its impact on families.
  • Computer literacy skills, including use of MS Office 365 and Teams.
  • Excellent communication, problem‑solving, and teamwork skills.
  • Effective verbal and written communication in English.
  • Ability to work effectively and safely without direct support or supervision in the community.
  • Availability to work Monday–Friday business hours; evening or weekend work may be required from time to time.
  • Current First Aid/CPR Level C is required;
    Suicide Intervention (ASIST) and Crisis De‑escalation training is an asset.
  • Police check and Child Intervention check are required for employment.
  • Valid driver’s licence (class
    5) and reliable vehicle.
