Position:Youth and Family Support Practitioner
Permanent 0.8 FTE (32 hours per week). Work days:
Friday through Monday, 7 a.m.–3 p.m. This role provides supervision, support, and direct care to youth residing in the Passages program and collaborates with the team to implement treatment plans.
The Passages Program is a six‑bed Live‑In program for youth ages 13‑17 with status with Children’s Services. It offers recreation, life‑skills and relationship‑based activities with an average stay of 6‑12 months, aiming to support each youth’s successful transition back to family or an appropriate placement.
Responsibilities- Provide supervision and support to youth in the program and ensure treatment plans are implemented.
- Assist with daily household routines, chores, and minor facility maintenance.
- Directly support youth through programs and activities.
- Maintain accurate computer records and complete daily administrative tasks including case management.
- Strong relationship development skills.
- Collaborative team player with creativity and strong organizational skills.
- Adaptive, flexible, and able to handle changing situations.
- Strong communication and de‑escalation skills.
Applicants must have at least a degree or diploma in a Human Service field and experience working with youth in a group‑living environment. Knowledge of trauma‑informed practices and the ability to work independently are required.
Other Qualifications- Current Standard First Aid and CPR
C. - Current Criminal Record Check with a Vulnerable Sectors designation.
- Valid Class 5 driver’s licence with a clean driving record.

All staff must complete an Intervention Record Check (IRC) and a Criminal Record Check including a Vulnerable Sector Check (VSC) before the employee’s start date. Checks may take up to six (6) weeks to complete and must be no more than six (6) months old at the time of hire.
