Family Supports Navigator at YW

Make a difference in the lives of children and families as a Family Supports Navigator with YW Calgary. This part-time position involves coordinating and advocating for community-based supports across Calgary.

The Family Supports Navigator provides crucial navigation, referral, and coordination services to families accessing the Family Resource Network. You will assist in identifying family strengths, overcoming barriers, and ensuring timely connections to vital resources. Collaboration with community partners is essential to support positive outcomes for children and youth in need.

Key Responsibilities:

• Complete intake and needs assessments for families

• Develop individualized service plans for ongoing support

• Facilitate referrals to appropriate FRN programs

• Follow up with families to ensure successful service connections

• Monitor barriers and advocate for coordinated responses

Requirements:

• Degree in Social Work, Child and Youth Care, or related field

• Minimum two years experience in community support roles

• Knowledge of child development and family-centric approaches

• Experience with case coordination and documentation

• Understanding of community service systems
