As a key member of the Community Wellness Services division, you will provide long-term support to residents. Your responsibilities will include conducting assessments and referrals, managing program delivery, and developing resources for community needs. A focus on networking and partnerships will be vital to increase awareness of available services and funding opportunities.
Key Responsibilities:
• Implement and coordinate community wellness programs
• Conduct client intakes and referrals to social services
• Develop resources and fundraising initiatives
• Report on program impacts and evaluations
• Facilitate training and mentoring for staff
Requirements:
• Bachelor's degree in social work or related field
• Minimum three years of progressive experience
• Registered with Alberta College of Social Workers
• Clear criminal record and vulnerable sector checks
• Valid driver’s license and first aid certifications
Deliver impactful social support through expert coordination, community engagement, and resource development as a Wellness Facilitator in Chestermere.
