Contribute to community wellbeing in Chestermere as a Wellness Facilitator within the Community Wellness Services division. This full-time position centers on enhancing social support access and fostering community inclusion.
As a Wellness Facilitator, your primary focus will be delivering professional support services to community members. Key tasks include coordinating programs, conducting assessments for referrals, and implementing initiatives aimed at poverty reduction. Collaborating with various stakeholders will be paramount to your success in addressing local needs and improving resource availability.
Key Responsibilities:
• Coordinate and enhance community wellness programs
• Conduct assessments and facilitate referrals for clients
• Develop partnerships and manage community initiatives
• Analyze program data for continuous improvement
• Guide and mentor CWS staff post-client interactions
Requirements:
• Bachelor’s degree in social work or an equivalent field
• Three years of relevant, progressive experience
• Registered with the Alberta College of Social Workers
• Background checks and valid driver’s license required
• First aid and mental health certifications needed
