In this Social Worker II role, you'll be integral to an interprofessional team, focusing on psychosocial assessment, individual and family counselling, and discharge planning. With assistance from Assisted Living Alberta, you'll ensure smooth transitions and empower patients and their families throughout their care journey. This position requires a dedicated professional ready to practice at full scope with a supportive network.
Key Responsibilities:
• Conduct psychosocial assessments for patients and families
• Provide individual and group treatment sessions
• Coordinate case management and transition planning
• Implement prevention, promotion, and awareness activities
• Utilize community resources to optimize patient care
Requirements:
• Bachelor’s degree in Social Work from an accredited institution
• 3-5 years of clinical experience in outpatient settings
• Registration or eligibility with Alberta College of Social Workers
• Ability to manage a fluctuating caseload independently
• Strong crisis intervention and documentation skills
Drive wellness and support in the outpatient setting with a focus on collaborative patient care.
