In this temporary, full-time position, you'll assist older adults in navigating resources and overcoming challenges. Working alongside families, colleagues, and community partners, you will assess needs, develop individualized service plans, and ensure clients receive appropriate care. Your goal is to empower participants for better social inclusion within their communities.
Key Responsibilities:
• Assess needs and create outreach service plans
• Connect participants to vital community resources
• Engage with families and community service providers
• Conduct presentations and contribute to team training
• Document participant interactions with precision and care
Requirements:
• Bachelor’s degree in social work or a related field
• 2-3 years of experience in community outreach
• Familiarity with social determinants of health
• Skills in problem-solving and collaboration
• Driver’s license and reliable transportation required
