In this full-time role, you'll oversee program success in Alberta's southern region, developing services that enhance wellbeing. Your background in service navigation and community resources will play a vital role in guiding clients through assessments and creating personalized support plans. Effective communication and relationship-building skills will deepen connections with clients, professionals, and community organizations.
Key Responsibilities:
• Provide information on disability services and supports
• Guide clients through personalized assessments
• Connect with community resources for clients
• Monitor and track program performance outcomes
• Foster collaborative relationships with stakeholders
Requirements:
• Post-secondary education in Social Work or equivalent
• 2-3 years’ experience in case management
• Knowledge of Alberta's disability support systems
• Strong interpersonal and organizational skills
• Commitment to inclusivity and continuous improvement
